We have a great opportunity for a full-time band 6 nurse to work in Intensive Support Element of the Dementia & Older People’s Mental Health Team based in Worthing West Sussex. Intensive Support operates an 8am – 7pm shift pattern, with a 1-hr break, Monday to Fridays ,9am – 5pm at weekends and bank holidays. On occasions the shift pattern may change to 9-5pm 7 days a week at short notice to meet any changes that may occur in the service.

You will join a friendly, highly-skilled and enthusiastic multi professional team. We will help you to develop the skills important to this role and you will be encouraged to develop special interests and expertise in line with service and team objectives. You will receive regular managerial and clinical supervision.

The post holder is expected to travel across the geographical area (AAW) covered by the team, the ability to travel to remote areas is essential

We will support the transition from Band 5 to Band 6

The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within their home and maintaining their wellbeing and independence as far as possible. We are looking for individuals who are dynamic, compassionate, good communicator who are able to demonstrate a holistic and person-centred approach to their work.

You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults.

Due to the needs of the service, there may be occasions when you will be required to work as a Lead Practitioner with a caseload.

Job responsibilities

Clinical

  • To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
  • To support/manage a team caseload and attend relevant MDT meetings
  • To regularly support and cover the duty rota
  • To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
  • To provide risk screening and assessment as well as risk management planning
  • To contribute to and facilitate evidence-based assessments and individual and group interventions
  • To contribute to the maintenance and development of the Service
  • To regularly share and supervise students on practice placement, and junior staff as appropriate

Knowledge/Experience

Essential

  • Post qualifying experience in the relevant care group
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working within and across various statutory and non-statutory teams and agencies
  • Experience of working within the CPA process
  • Understanding of clinical governance
  • Previous experience of working in the community

Qualifications

Essential

  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development
